29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident a. On or about January 04, 2015, the employer failed to inform OSHA within 8 hours after the death of one of their employees from a work related accident. The employer was advised of the victim's death at approximately 5:00 PM EST on January 4, 2015, and OSHA was notified by the Human Resources Director at approximately 11:45 AM EST on January 7, 2015.
