Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards inherent to mechanical motion of the single head chime sealer machine located in the Open Head Department as the result of not providing appropriate lockout procedures such as: Among other methods, one feasible and acceptable abatement method to correct this hazard is, but not limited to: 1) Ensuring that locks keyed separately and/or multiple locking devices be provided and used where more than one person is involved in operations requiring a lockout: 2) Ensuring that equipment is rendered safe (either by the use of electrical test equipment or by attempting to activate the equipment) and, 3) Ensuring that regular enforcement and periodic evaluation of lockout policies are made to determine usage and adequacy of the procedures.
