29 CFR 1910.1450(e)(1): Where hazardous chemicals were used in the workplace, the employer did not develop and carry out the provisions of a written Chemical Hygiene Plan which is to be capable of protecting employees from health hazards associated with hazardous chemicals: (a) Location - Establishment On or about 1/7/15 the employer did not carry out the provisions of the Chemical Hygiene Plan by performing annual hood evaluations. (b) Location - Establishment On or about 1/7/15 the employer did not carry out the provisions of the Chemical Hygiene Plan by evaluating the ventilation efficiency of glove boxes, gas cabinets, and other ventilation engineering controls (protective equipment) to ensure proper and adequate performance.
