29 CFR 1910.23(a)(2): Ladderway floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could not walk directly into the opening: Employees were exposed to fall hazards at Unit 6 Grade Floor by the 62 Boiler because: a. the ladderway platform opening at 470' elevation for the 622 GR Fan was not guarded by a standard railing. Employees were exposed to a fall distance of approximately 6.2 feet. b. the ladderway platform opening 483' elevation above the 621 Economizer Tank was not guarded by a standard railing. Employees were exposed to a fall distance of approximately 13.8 feet.
