Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries: a) On or about January 15, 1987, employee(s) were not trained in an effective method to positively lock-out the Challenge cement mixer during servicing/maintenance operations to prevent crushing injuries to employee(s). One feasible and acceptable method of abatement to correct the hazard, among others, is to positively lock-put the equipment's main power source, and train all employees in the safe procedures to be followed.
