Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the recognized hazards of methane, carbon monozide, hydrogen sulfide (H2S), and natural gas associated with raw sewage: a) Employees working in the Wet Well Area of the Headworks Building had no means of notification when combustible gases reached levels which necessitate evacuation. The permanently affixed Combustable Gas Monitoring Alarm System (MSA model no. 51501) was inoperable for approximately 6 months prior to and on 11/14/89. Among other methods, one feasible and acceptable abatement method to correct this hazard is to: 1) Repair or replace monitoring system. 2) Train employees to interpret and understand the warning system. 3) Educate and train employees as to what actions to take in an emergency situation.
