29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: In the alternative: 29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a.) Alley area. Employee(s) were exposed to falls of approximately 4 feet 6 inches from the working platform of the flatbed trailer on the 1997 International truck, while unloading rolls of paper weighing approximately 1445 lbs., with no fall protection, as observed on or about December 17, 2014. ABATEMENT DOCUMENTATION IS REQUIRED.
