Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries from unintentional movement of mechanical equipment, severe electrical shock. a) The Employer failed to implement a positive lockout procedure to protect employee(s) performing maintenance on electrical and mechanical equipment in the cooling tower located on the roof level of the building at 1331-17th Street, Denver, Colorado. A feasible and useful method to correct this hazard among other methods is to establish and enforce standrad operating procedures which would include but would not be limited to: a) Lock-out procedures including assignment of individual locks to all personnel involved in servicing/maintaining equipment. b) Training procedures/methods of all personnel involved in lockout procedures.
