Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: On or about 1/4/89, industrial truck operators were exposed to serious physical injury when loading delivery vans equipped with hydraulic lifts that could not be secured to the ICC dock lock system. Policy was not adequate to inform the operators on how to identify the vehicle and to recognize the hazard of vehicle separation from the loading bays. Among other methods feasible and acceptable abatement methods to correct the hazard are: 1) Identify the vehicle cargo interior and compartment doors. 2) Provide training that emphasizes the identification of the rear hydraulic lift gate delivery vans. 3) Instruct the industrial truck operators of their responsibility to have the vehicle chocked and brake set. 4) Establish a written "docking procedure" and strictly enforce it. 5) The written procedure should include the removal of the vehicle key from the ignition, and maintaining it in view of the industrial operator as he is loading the cargo compartment.
