Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recgonized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Inadvertent activation of molding line equipment and related material transfer mechanisms while freeing flasks from jams, making adjustment in hazardous machine locations, and troubleshooting in hazardous machine locations. Among other methods, one feasible and acceptable method to correct this hazard is to implement, enforce and train operator(s) involved in clearing jams, making adjustments or troubleshooting problems on sand molding line machinery in the company's previouslydeveloped "Maintenance and operating 'Lockout' Procedure." a) Blow mold machine plant #2 punch out and adjacent shuttle conveyor. b) Plant #1 blow mold machine mold machine and flask pusher.
