Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about 4 October 1989 and at times prior to, employees were exposed to crushing injuries while cleaning the Anets Dough Roller, Model Number SDR-21 in that the safety interconnects had been by-passed and no lockout procedure was employed to ensure that the system was de- energized prior to removing the guards. One feasible method of abatement among others would be to establish a lockout procedure and to train all employees in that procedure and to follow manufacturers recommended cleaning procedure, and to immediately repair the machine so that rollers will not function with the safety interconnects removed.
