Text For Citation: 01 Item/Group: 001 Hazard: UNAPEQUIP
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about September 15, 1989 employees were exposed to the hazard of using/supplying a relief valve attached to the "Gardner-Denver" Engine Driven Pumping Unit Molel FDFXX-B, serial no. 104164, located at the Apache Canyon Salt Water Disposal Well No. 19-10 in Las Animas Country, Colorado; whose connecting threads were not "made--up" to the minimum recommended distance of at least 5/8 inch. Among other methods, one feasible and acceptable abatement method to correct this hazard is to ensure all pressure pipe of this diameter is is "made-up" before use of at least 5/8 inch as per recognized industry practice.
