Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) Death and crushing injuries caused by the overstressing of the temporary welds on the column at W3-N48. On or about 02-25-92 temporary welds at the base of the column at W3-N48 were overstressed due to the load imposed on them from the tensioning force in the guy wire applied by a come-along and an additional force applied by an ironworker's sleever bar. The steel erection company did not determine the approximate force needed to move the top of the free standing column into place. Among other methods, one feasible and acceptable abatement method to correct this hazard is: Ensure that columns and similar structural members are secured and braced from movement to prevent collapse.
