29 CFR 1915.77(c): When employees were working aloft, or elsewhere at elevations more than 5 feet above a solid surface, the employer did not provide either scaffolds or a sloping ladder, meeting the requirements of this subpart, to afford safe footing, and the employees were not protected by safety belts and lifelines meeting the requirements of 1915.159 and 1915.160: a) Tiffany Yachts, Incorporated, Boat Shop, 2355 Jessie DuPont Memorial Hwy, Burgess, VA - On or about 11/25/14, an exposed employee was working from the upper deck of a boat without a scaffold, sloping ladder, safety belt or lifeline and fell approximately 6 1/2 feet onto the concrete surface below, resulting in fatal injuries to the employee. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
