Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being struck by large industrial trucks that are permitted to operate in reverse gear without any audible warning device while in the immediate work areas of employees: a) Tioga Marine Terminal - a 30,000 pound Silent Hoist Crane Company Forklift truck number 2135, having an obstructed view to the rear, was permitted to operate in the immediate work area of employees in reverse gear without giving any adequate audible warning. Observed 4/18/88. *Abatement Note: Among other methods, one feasible and acceptable method to correct this hazard would be to install reverse signal alarms and to ensure they are maintained in an operable condition.
