29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about November 14, 2014 in building #2, east wall, the auger grate on the mixer bin was not designed and constructed to prevent any part of the body from being in the danger zone during the operating cycle. The employees were exposed to caught-in hazards.
