Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: (a) On or about September 14, 1992, at Associated Natural Gas Inc. plant in Gilcrest, Colorado, employees performing start up procedures on the Petrotherm Gas Fuel Heater Model #H-70495 were not protected from the hazard of the flying object, caused by an internal explosion of the heater, due to the menway door, containing the inspection hole, was not bolted shut. Among other methods one feasible and acceptable abatement method to correct this hazard is to bolt the door shut and conform with standard recommended engineering requirements of the most recent ASME Boiler and pressure Vessel
