Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: On or about November 9, 2014, employees were exposed to the hazard of being struck-by/crushed when working on the drilling platform within the vicinity of elevated equipment, such as oil well test pipe suspended by elevators. Among other methods, one feasible and acceptable method to correct this hazard is& 1) Inspect the elevators before use to ensure employees are always selecting and using the appropriately sized elevator for the pipe being lifted; per API RP 54, p.19, Sec.9.7.8. 2) Communicate to employees the policy prohibiting access to the area underneath suspended loads, and enforce the policy through an effective disciplinary program.
