Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught-in-between hazards. On or about November 5, 2014, adjacent to the airplane hanger at 782 Jasmine Rd., Crowley, LA. an employee was exposed to a crushing hazard when he was directed to unchain the connection between the bucket of a Link-Belt 210 X2 excavator and a frac tank. At the same time as he stood between these two objects a manager approached the cab of the excavator to have a discussion with the operator. The excavator was not de-energized or disabled. As a result, when the operator in the cab stood up and accidentally made contact with the joystick controller the boom extended and crushed the employee between the bucket and frac tank.
