29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about 11/06/2014, at the above mentioned job site, employees were exposed to struck-by hazards while using a forklift with an attachment installed to its' mast to lift fiberglass boards and written approval from the manufacturer for the attachment was not received.
