Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) The hazard of an employee becoming caught between any of the 4 operating compartment plows, front and rear corner positions, and any of the structural building columns, or accidentally falling across a rail cog and being struck by any of the 4 operating compartment Plows in Malthouse #1. Among other methods, feasible and acceptable methods to abate the hazard would include but not be limited to: - Incorporate and administer a policy requiring th Plow Operator post to post a barrier with a warning sign to prevent unnecessary Malthouse employee entry. The policy shall limit the access of employees to one person to assist the Plow Operator in the "Mucking Out" operation. - The policy shall require the assistant to visually report to the Plow Operator. The policy shall also include the requirement of visual contact between the Plow Operator and the "Mucker" at all times. - Installation of a large convex mirror at the Plow Operator's off- side to provide a view for the Operator's blind side. - Installation of an effective warning signal on each compartment Plow in the form of an audible bell or horn and a visual light to indicate when the Plow is in operation. - Installation of identification labels on all 4 Plow Operator's control levers and electric switches including the jack up and down buttons.
