Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On 9/1/93, 32 South BCP, 34 feet elevation, Ravenswood Employees attempting to unseat a stuck valve were exposed to the hazard of being struck by parts of the valve, valve controller and/or the equipment which was applied to the hand wheel in an improper attempt to force the valve open. Employees could suffer severe fractures, lacerations, internal injuries or death. ABATEMENT NOTE: Among other methods, one feasible and acceptable method to correct this hazard is to: 1 - Follow manufacturers' instructions for the operation and maintenance of all equipment used.
