29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer: a) When employees are exposed to falling objects, the employer does not ensure that frequent and regular inspections of the job site are made by a competent person to ensure that employees do not enter into areas where objects can fall. This violation most recently occurred on or about October 15, 2014, at the construction site in Dallas, Texas; where employees were not protected from falling objects while an excavator loaded dump-trailers.
