Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious injury from being caught between closing machine rolls: a) Employee work inside opened roll sections of the Ward Flexo-Folder- Glue Machine (company #6010) when the power switch was not positively controlled with an effective lockout system, May 9, 1988. Among other methods, one feasible and acceptable abatement method to correct this hazard is to establish and enforce an effective lock-out tag-procedure.
