Text For Citation: 01 Item/Group: 001 Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing injuries from railroad car hopper doors which were being opened and closed without ensuring that door areas were closed. Among other methods, one feasible and acceptable method to correct this hazard is to: a) Institute and enforce a written lock-out procedure whereby the maintenance employees would physically lock out the control box to prevent unintended operation of the door, and b) Institute and enforce a written procedure requiring visual contact between the door operator on one side of the car and the laborer on the opposite side of the car, so that both employees would stand at the end where the controls are, with one employee on each side of the car.
