Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On or about December 11, 1985 the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of being caught between the descending or ascending power freight elevator and the elevator shaft wall or the horizontal sliding doors. Elevator located at the North End of the First Floor Production Area was used by employees to transport plastic material to the third level Banbury hopper feed area. Among other methods, one feasible and available method to correct this hazard is the installation of a hoistway-unit system hoistway-door interlock.
