29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment or walls) were not protected by a cover that leaves no openings more than 1 inch wide: a) 72 Lockwood Street, Newark, NJ The 501 tank is surrounded by a 16 foot high platform with a gap of 2 inches between the platform and the toe board surrounding the tank through which equipment can fall to include but not limited to a wrench used to open the vessel's hatch. Violation observed on or about 10-15-14 Abatement Note: The cover shall be securely held in place to prevent tools or materials from falling through.
