Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On January 9, 1987 at or about 1:15 p.m. employee(s) were subjected to the hazards of being crushed and/or death while erecting precast concrete columns, beams and planks. In that a T-shaped ledger beam was eccentrically loaded without first restraining the member from rotating. 1) Written procedures covering stability and equilibrium of the structure and componenets during all phases of construction. 2) Beams must be locked into place using welding and grouting before plank can safely be set on them. 3) Temporary restraint must be provided during erection. 4) Training of any and all employees required to work in the area during the erecting state. Such training shall include but not be limited to considerations and procedures pertaining to the stability and equilibrium of the structure and its componenents during all phases of construction.
