29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator: The employer does not protect each employee during basement cleaning by providing fit testing. (a) On October 10, 2014, employees required to wear N95 half mask filtering face piece respirators while cleaning the basement sand reclamation area were not fit tested. (b) On or about October 20, 2014, an employee required to wear N95 half mask filtering face piece respirators while cleaning the basement sand reclamation area were not fit tested on an annual basis. (c) On October 21, 2014, an employee required to wear N95 half mask filtering face piece respirators while cleaning the basement sand reclamation area were not fit tested. (d) On October 22, 2014, an employee required to wear N95 half mask filtering face piece respirators while cleaning the basement sand reclamation area were not fit tested. (e) On or about October 27, 2014, an employee required to wear N95 half mask filtering face piece respirators while cleaning the basement sand reclamation area were not fit tested on an annual basis. (f) On or about October 28, 2014, an employee required to wear N95 half mask filtering face piece respirators while cleaning the basement sand reclamation area were not fit tested on an annual basis. (g) On October 29, 2014, an employee required to wear N95 half mask filtering face piece respirators while cleaning the basement sand reclamation area were not fit tested. To abate this hazard in the future the employer must provide respirator fit testing to employees required to wear respirators prior to initial use and annually thereafter.
