Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: potential serious or fatal injuries by being crushed between the casing being raised by the Mereco hydraulic laydown machine and the elevators or the casing just lowered in the hole. A feasible and useful method to correct the hazard is to install an emergency shutdown switch on the rig floor to enable the spotter to shut the machine down and prevent the casing from hitting the employees: a) Rig #7, Endicott Sattelite Island: The employer did not provide an emergency shutdown switch on the rig floor exposing employees to crushing injuries on June 27, 1986.
