Investigation Summary
On July 18, 2009, Employee #1 was installing coax cable hangers down the North face of a guyed tower. He was climbing down the tower cross members installing hangers on six newly installed coax cables at approximately 47-inch intervals, when he fell between 60 and 100 feet and was killed. Further investigation revealed that he was wearing a full-body harness with a 6-foot shock-absorbing nylon web lanyard but was not using it at the time of the accident.
