Investigation Summary
Employee #1 was working with a crew pulling up cables on a communication tower. He was approximately 150 feet up the 300 foot tower. His job was to secure the cable with a hoisting grip and ground it after the cable was set at the top of the tower. Two cables had been pulled into position and secured and the third cable was just starting to be pulled up when the employee fell to the ground and was killed. The investigation revealed that Employee #1 was only using a positioning belt along with his harness. His lanyard was in the tool box on the ground at the time of the accident. It is believed that the employee disconnected his positioning belt to move out of the way of the cable being pulled up and fell during his attempt to move.
