Abstract: At approximately 9:30 a.m. on March 18, 2017, Employee #1, a banquet cook, was working in the banquet prep area of the kitchen preparing food. Employee#1 was walking out of the walk-in cooler with supplies to start cooking for a banquet. Employee #1 approached his utility cart to put supplies down and bumped the right side of his head on the shelf by the hand washing station. On the same day, Employee #1 reached for saucepots on a top shelf of a storage rack when one of the pots on the top shelf fell and hit Employee #1 on the right side of head. Employee #1 continued his shift. No one was working in the kitchen nor observed the accidents. Employee #1 did not report the accident to management during or after the work shift. On March 19, 2017, Employee #1 woke up with a very bad headache, and notified his employer that he would not be coming into work and went to the hospital. At the hospital, Employee #1 was treated for a head injury and immediately transported by helicopter to another hospital where Employee #1 was diagnosed with subdural hematoma, bleeding on the brain and underwent brain surgery.
