Abstract: At 1:00 a.m. on May 15, 2017, an employee was cleaning meat from Pre-Blender #3 and was using a metal hook to reach the paddles at the top of the blender and the discharge auger below the paddles. The employee tried saving the hook from being wrapped in the auger but trapped his fingers when the hook handle smashed into the safety guards on top of the pre-blender. The employee sustained two open fractures to their right ring finger and their right pinky. Amputation was not necessary; however, surgery was scheduled six weeks from the date of injury.
