Abstract: At around 1:30 p.m. on February 6, 1992, Employee #1 and his crew were called to assist another crew at Vaca Commons in Vacaville, CA. They brought a model SS material lift handling truck, which has a higher reach basket, to the job site. To use the basket, the front and rear tie down straps have to be released. While standing on top of a tool bin, Employee #1 tried to loosen the front tie down strap by pushing the ratchet bar. His feet slipped off the tool bin and he fell about 8 ft to the ground. Employee #1 was taken to Vaca Valley Hospital, suffering a shattered right ankle and broken tibia.
