Abstract: At 6:45 a.m. on April 12, 2017, an employee was working with a coworker to replace the hopper floor of a garbage truck. As they used a hydraulic floor jack to straighten out a bowed metal plate in order to weld a new piece in its place,the jack saddle extension shaft started losing contact with the bucket frame. The employee grabbed hold of the extension shaft, resulting in his right thumb becoming caught between the jack extension shaft and shim. The tip of his thumb was amputated, which he was hospitalized and received treatment.
