Abstract: At 6:30 a.m. on April 22, 2017 Employee #1, a journeyman ironworker, was working at a pier to perform steel fabrication and welding repairs. As Employee #1 was working on a conveyor, using a Rose Bud (heating) style torch, an explosion occurred when he opened the oxygen valve that fed the torch. Employee #1 sustained burns to his abdomen. There were no witnesses to the incident, and forensic analysis of the torch produced no indication as to why the explosion occurred.
