Abstract: At approximately 3:45 a.m. on April 28, 2017, four workers, including an employee, were performing routine maintenance on a primary shredder. The workers utilized lockout procedures to safely de-energize the rotating equipment before removing its housing. Employees successfully removed three of four hollow pins that locked hammers in place by inserting threaded rods "all-thread" into the pins and pulling them out using a ratchet. The fourth pin proved difficult to remove, and following SOP, workers used the "all-thread" and inserted it into the hollow pin. But due to thread wear, workers were not able to remove the fourth pin. Following SOP, a fifth worker welded the all-thread to the pin before the employee operated the ratchet to slowly remove the pin. The all-thread failed at its weld, but due to a restraint (rigging strap), the all-thread was prevented from impaling the employee. However, the restraint caused the all-thread to swivel, causing the welded end to strike the employee's left thumb, resulting partial amputation of left thumb. The employee was hospitalized and treated for his injuries.
