Abstract: At approximately 12:45 a.m. on June 19, 1991, Employee #1 was feeding two strips of approximately 15 in. long by 1.7 in. wide by 188 in. thick press board into a Newman press board crimping machine. The crimping machine has two 12 in. long by 6 in. diameter cylindrical rollers placed horizontally with crimping grooves running lengthwise on the rollers. The machine will accept two to three strips of press board at one time, side by side. When Employee #1 fed the two strips into the machine, they overlapped. He stopped the machine and set it in reverse so that he could remove the strips from the roller's grooves. The employee then went to the rear of the feed side, took another piece of press board, and tried to clear the grooves. His left hand became caught in the roller's pinch point and several of his fingers were amputated. The machine's infeed is well guarded when the machine is in the forward mode.
