Abstract: At approximately 1:00 p.m. on April 6, 2017, Employee #1 was working on an inclined batch off conveyor system within the Number 17 mixer basement building of an automotive tires manufacturer. He and a coworker, both subcontractors for a rubber machinery company, were working with electrical and pipefitter craftsmen to install a new rubber mixing machine. A twin-screw rubber (TSR) mixing machine was being replaced. Employee #1 was removing a chain ratchet from equipment between the compressed conveyor and the TSR mixing machine. The conveyor unexpectedly moved into the extended position and struck and pinned his torso between the conveyor and the TSR mixing machine. Factors that may have caused or contributed to the incident include the lack of energy controls for the conveyor. Employee #1 suffered crushing injuries to the chest, back and pelvis, including a bruised lung, and fractured rib and hip. Employee #1 was transported to a medical center, where he received treatment and was then hospitalized.
