Abstract: At 9:45 a.m. on January 28, 2017, an employee and a coworker removed the guard covering the chain drive so the new motor could be bolted on. The employee adjusted the chain to ensure it was tight while the coworker was wiring the motor. The coworker then went upstairs to remove the lock and restore power to the motor. The employee remained in the basement area to determine if the motor was running in the correct direction. The employee told the coworker that the motor was not running in the correct direction, so the coworker reapplied the lock and came back down to the basement area to rewire the motor. The coworker then went back upstairs and removed the lock again while the employee remained in the basement to observe the motor. The employee told the coworker that it was running in the correct direction. The employee then placed his left hand on the chain to make sure it was tight. At that same time the coworker turned the motor on again. The employee's left hand was on the chain drive which operated at 156 rpm. The employee's left fingers were pulled into the chain and partially amputated.
