Abstract: At 5:30 a.m. on January 9, 2017, an employee was attempting to remove a T-shirt that got wrapped around the conveyor roller causing the limit switch to indicated a tracking fault and stop the conveyor. The employee was using a utility knife and reaching behind the conveyor belt to cut the T-shirt and remove it. As pieces of the T-shirt were removed the roller was able to move away from the limit switch which allowed the conveyor belt to continue running, since the unit was not locked out. The employee's left had was caught between a roller and the conveyor belt, resulting in third degree burns to the back of his left hand and fingers which required skin graphs.
