Abstract: An electrical contractor had contracted with an electric cooperative to perform construction work on an overhead power line. The work included adding two new phase conductors to the existing line, replacing poles and hardware, and replacing the existing conductors with 336 ACSR conductor. Two power line workers were working from two aerial lifts positioned on either side of a pole. They were dead-ending a newly installed conductor. The two employees had worked for a number of years, allegedly as experienced power line workers. Despite this, neither employee was wearing rubber insulating gloves and sleeves, nor had they covered any of the adjacent conductors with electrical protective equipment. Both employees were electrocuted while they were performing this task. (The original form did not indicate exactly how the accident occurred.)
