Abstract: At 10:00 a.m. on January 9, 2017, an employee was at an Air Force base and working from an elevated platform. While he was using a hoist to bring a metal box up to the platform. The platform had a gate, which was open so that the metal box could be brought onto the platform. The employee was holding the remote control for the hoist in his hand. The employee did not let go of the 'up' button on the hoist, and the hoist wire broke. When the employee attempted to grab the metal box, he fell approximately 5.8 to 6.1 meters (19 to 20 feet) to the ground. He sustained multiple fractures, including broken feet, broken ankles, and a broken tailbone (coccyx). He also injured his right arm and elbow.
