Abstract: Two corrugator operators were clearing a breakout (where only lining, no cardboard, comes through the machine) and had completed feeding the cardboard through the slitter when they noticed Employee #1 caught in the rollers of the machine. There were no eyewitnesses, but it appeared that the employee had attempted to catch the strapped lining, which had been cut and lapped over the drum, before it went into the rollers after the corrugator had been started. Both of his arms were pulled into the rollers through the ingoing nip point. Employee #1 died three weeks after the accident, after having both arms amputated.
