Abstract: At 4:30 a.m. on September 12, 2016, an employee, a truck driver was attempting to manually raise and retract the landing gear on a flatbed trailer. He had just completed connecting the electrical and air lines from the tractor to a flatbed trailer. The trailer was parked next to another flat bed trailer with approximately 7-in. between. The tractor was angled approximately 45 degrees from the trailer. As the employee began to pull away with the trailer attached, he looked in the passenger side mirror and noticed that the trailer landing gear was still in the down position. The employee exited the tractor without setting the parking brake and walked between the two flat bed trailers to access the manual crank located under the trailer that retracts the landing gear. The employee began to turn the crank which placed his head at the same level as the bed of the trailer. The tractor and trailer rolled backwards as the landing gear lost contact with the ground. The employee's head was caught in and crushed between the two flat bed trailers. He was killed from severe head trauma.
