Abstract: On June 12, 1993, Employee #1 was at an interstate carrier's maintenance shop. A tractor truck was backing up to couple its fifth wheel with a reefer semi-trailer. When the trailer dolly was lifted, Employee #1 was found between the trailer's kingpin and the fifth wheel. He had been killed. The reefer unit was on. The tractor did not sound the the air horn before backing up or follow standard fifth wheel coupling procedures.
