Abstract: From 1:30 p.m. to 3:30 p.m. on November 14, 1988, Employees #1 and #2 were organizing beer kegs and pallets in a keg box. Employee #1 had come off his job as a truck driver to assist Employee #2, who was operating a propane powered forklift, Click GES 25 MB, type LP, serial #6138 MB 0202-6221 FA. Employee #1 was in the keg box for the entire duration of the task. Employee #2 was operating the lift for the entire duration; he was in the keg box approximately 70 percent of that time and during the other 30 percent was retrieving orders for the keg box. Between 2:30 p.m. and 3:30 p.m., both employees developed symptoms of carbon monoxide poisoning. Employee #2 merely stepped outside to rest but Employee #1 felt extremely ill and reported to his supervisor at 3:40 p.m. for admission to Shady Grove Adventist Hospital. The two employees were not aware of the hazard because they had received no training with regard to carbon monoxide or propane. They were not offered ppe or carbon monoxide monitors.
