Abstract: At approximately 10:00 a.m. on March 21, 1991, Employee #1, a maintenance man, and a plant coworker, were taking measurements on a log cutoff saw for a merchandiser. They prepared to go to the other side of the equipment for some more measurements and had climbed out over the railing and proceeded to the other side of the catwalk. The employee elected to cross over the log pan kickers instead of walking around. As he crossed over the log pan, he activated the beam on an electronic eye, starting a chain reaction, and causing the log pan to raise at a 90 degree angle and try to empty. The employee slipped and fell as the machine went through its cycle and started to return. He was almost out of the machine when the log pan closed, catching his left foot and crushing it. The foot was later amputated at the hospital. Employee #1 had not locked out the machine.
