Abstract: At approximately 1:00 p.m. on June 16, 2015, Employee #1 was working in a trench that did not contain a trench box. A large portion of the trench caved in trapping and burying him in soil to the waist. Employee #1 suffered massive blunt force trauma, fractured legs, and a massive cut across the pelvic region which caused him to bleed out and Employee #1 died.
